			<content:encoded><![CDATA[<p style="text-align: justify;">Overweight and obesity are linked to a number of negative health consequences: the risk of heart disease, osteoarthritis and diabetes are much greater in those who are obese than in normal-weight people. While weight loss and healthy weight maintenance may be achieved by alterations in lifestyle, such long-term changes can be difficult for many. <span id="more-86"></span>Thus no prescription weight loss drugs (also called over-the-counter or OTC products) appeal to those who struggle with weight problems. According to a report in the August 23 issue of the Journal of the American Medical Association (JAMA), little is known about the characteristics of the people who use such products, whether they are used in conjunction with prescription medications, or if they are used instead of or in addition to lifestyle changes.</p>
<p style="text-align: justify;">Heidi Michels Blanck, Ph.D. and colleagues from the Centers for Disease Control and Prevention (CDC) in Atlanta examined data on OTC and prescription weight loss products collected by five state health departments from 1996-1998. They were particularly interested in use of products containing phenylpropanolamaine (PPA), and the herbal product ephedra. PPA has since been withdrawn from the OTC market because of indications that it might increase the risk of stroke among users, and ephedra has been associated with numerous adverse health effects.</p>
<p style="text-align: justify;">Respondents to the telephone surveys included 8,133 men and 8,546 women over 18 years of age. In addition to information about their use of these products, the subjects also specified their height, weight, dietary intake, typical physical activity level, and presence or absence of diabetes.</p>
<p style="text-align: justify;">The researchers report that 7 percent of the subjects reported using at least one OTC weight loss product in the previous 2 years; women and younger people were more likely to report such use, as were obese individuals. Two percent of the subjects used an OTC product containing PPA, and 1 percent used products containing ephedra. Over one third of respondents who reported taking <a href="http://www.drugsboat.com">prescription drugs</a> for weight loss reported using some type of OTC product as well.</p>
<p style="text-align: justify;">OTC product use was not correlated with intake of fruits and vegetable, but the investigators did find that respondents who reported at least some physical activity were more likely than inactive people to use OTC products. Of the subjects who reported that they were diabetic, 5.9 percent said that they had used an OTC weight loss product. In addition, 8 percent of normal weight women used OTC weight loss products.</p>
<p style="text-align: justify;">Government statistics document an alarming increase in the prevalence of obesity among Americans over the last two decades. The National Institutes of Health report that over one half of adults are overweight, and nearly one quarter are obese. Dr. Blanck and her coauthors estimated that, based on their data, between 1996-98, 17.2 million Americans used OTC weight loss products, 5 million used procucts containing PPA, and 2.5 million used ephedra products.</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">One of the most common questions I am asked is “Does acupuncture work for weight loss?” The answer to this question is yes, but does require further explanation.</p>
<p style="text-align: justify;">First, acupuncture is part of TCM (Traditional Chinese Medicine). In my practice I use both herbs and acupuncture to help my patients. <span id="more-78"></span>These two tools complement each other very nicely. The acupuncture works more from the”outside in” while the herbal formulas work from the “inside out”. use both for weight loss to maximize the effects of the treatment. Second, there are appropriate and inappropriate uses of Chinese Medicine (particularly herbs) in dealing with weight loss.</p>
<p style="text-align: justify;">TCM is based on a framework within which a patient’s pattern of disharmony is defined. The treatment is designed within the same framework to restore harmony and balance to the individual. This pattern discrimination and corresponding treatment is the essence of TCM.</p>
<p style="text-align: justify;"><strong>Inappropriate Approaches</strong></p>
<p style="text-align: justify;">There are herbal weight loss “cocktails” which contain Chinese herbs. Unfortunately, these are marketed with a “one size fits all” mentality disharmony. As a professional herbalist and TCM practitioner, I consider this an EXTREME ABUSE of herbal products.</p>
<p style="text-align: justify;">Not only is this dangerous for the person taking the formula, but it also gives TCM a “black eye” as the public (and FDA as well) view Chinese herbs synonymous with TCM. These herbal cocktails typically contain herbs that will do one or a combination of the following three things.</p>
<p style="text-align: justify;">(1) “Purgatives” which attack and downwardly drain the food we eat causing a loss of essential nutrients.</p>
<p style="text-align: justify;">(2) “Diuretics” which cause seepage of fluids and can lead to electrolyte imbalance and ultimately kidney problems. (3) “Diaphoretics” which cause sweating. In TCM they will “release of the exterior” and cause Yang Qi to rise. Prolonged use leads to Yin damage.</p>
<p style="text-align: justify;">Each of these strategies is appropriate for specific imbalances. Purgatives for certain types of food stagnation, diuretics for certain patterns of fluid retention, and diaphoretics for “wind cold” types of the common cold. But when applied in combination to someone without the appropriate pattern, these strategies can do permanent damage.</p>
<p style="text-align: justify;"><strong>Appropriate Approaches</strong></p>
<p style="text-align: justify;">If prescribed and used properly, Chinese herbal formulas should be harmonious within themselves, creating no side effects or long term damage. An effective formula that produces no side effects is the reflection of a skilled practitioner. Appropriate TCM approaches to weight loss focus on the pattern of disharmony underlying the weight gain. A treatment should include modification of diet and activity as warranted, as well as acupuncture and herbs. All should be tailored to address the disharmony. General TCM Dietary Recommendations.</p>
<p style="text-align: justify;">There typically are four primary and three secondary patterns of disharmony associated with excessive body weight. There is a unifying feature of these patterns in that they all involve the spleen. In TCM the spleen is the “transformer and transporter”of the Qi energy that comes from the food we eat. In all cases the spleen is deficient in some fashion. So the spleen becomes the center of the treatment. I use the following dietary recommendations for anyone with spleen qi vacuity. The goal is to maximize warming foods and minimize cold foods. (The temperature of food refers to their internal effect on the human body, not to their temperature as we would normally think about it, although their physical temperature does play a role).</p>
<p style="text-align: justify;">1. Eat frequent, small meals</p>
<p style="text-align: justify;">2. Cooked food is preferable to uncooked food &#8211; Eat plenty of vegetables, rice, noodles, soups and stews Eat small amounts of chicken or turkey, especially in soups. Grains should be thoroughly cooked for easier digestion. Vegetables should be lightly cooked as excessive cooking results in loss of nutrients. Drink the water from cooked vegetables as a broth.</p>
<p style="text-align: justify;">3. Drink warm water, soup, broth or herbal tea with your meals.</p>
<p style="text-align: justify;">4. Snacks can include sunflower nuts or walnuts (most other nuts are less desirable).</p>
<p style="text-align: justify;">5. Use warming spices such as pepper (black or white), cardamom, ginger (fresh or powder), cloves, nutmeg, orange peel and fennel in food preparation (usually recommended, but may need to be modified if you have mixed heat and cold in your pattern of disharmony).</p>
<p style="text-align: justify;">6. Foods with a cold nature damage the spleen and should be avoided/minimized. Avoid/minimize raw foods such as salads, fruits and vegetables. Avoid/minimize fruit juices. Avoid/minimize chilled or frozen beverages, treats, etc (these are especially bad with meals).</p>
<p style="text-align: justify;">7. Avoid/minimize sugars &amp; sweets.</p>
<p style="text-align: justify;">8. Avoid/minimize dairy products.</p>
<p style="text-align: justify;">9. Avoid/minimize beer (Hops are very cold in nature.)</p>
<p style="text-align: justify;">10. Be cautious with Echinacea, Goldenseal and mega doses of Vitamin C (all are believed to be cold)</p>
<p style="text-align: justify;">Comprehensive treatment would involve a combination of dietary therapy, exercise, herbal medicine and acupuncture. Weight loss according to TCM is not easy, no healthy weight loss program is. <p style="text-align: justify;">There is a trend to use fewer meats, but some meat is necessary unless you are extremely careful with food choices and food combining. A little meat makes complete nutrition fairly easy.</p>

<p>You are beginning a unique format for nutrition counseling. It appears you have been on a number of diets before, and even pills. Could you please give me a history of your weight? How much did you weigh at age 16? What was your weight before the first pregnancy? What kind of programs and what kind of results have you tried? What appealed to you about these programs? What diet pills have you used and how much weight did you lose? Did you exercise while you were on these programs? The weight history tells me how you are motivated, what works for you and what doesn&#8217;t, and how your body responds to calorie restriction.<span id="more-64"></span></p>
<p>Please fill out a food diary. Your brief description of what you eat did not appear unreasonable. However, I don&#8217;t know how large the portions are or what kind of soup, etc. you eat. More detail will help me do a better job of helping you. It sounds like you try to snack on apples and veggies. How many cookies and chips do you have and does this happen daily?</p>
<p>So, keep a daily log of your food intake and send it in each Saturday. I won&#8217;t ask you to do this for the length of the program, but it&#8217;s necessary to get started. If you have just recently changed your eating when you started exercising four weeks ago, tell me how you ate before that time.</p>
<p>After I review a week of your eating, I&#8217;ll make recommendations. Report your weight each week, and feel free to make comments about your successes and challenges.</p>
<p>I look forward in supporting you in your weight loss efforts.</p>

<p><span id="more-56"></span>Answer:  We don&#8217;t like to recommend taking diet pills to anyone. Unless you are morbidly obese and under a doctor&#8217;s supervision, there are better ways to lose weight. Diet and exercise is the age-old maxim for losing weight. If the body takes in more calories than it works off, it stores the excess as fat. There is no magic pill.</p>
<p>If you are serious about losing weight, begin an exercise program and modify your diet. We know. It sounds easy. It is not. That&#8217;s why there are so many diet hucksters out there selling instant weight loss to hopeful fat people. Start with easy walking and increase time and distance in small increments each week. For instance, start with 10-minute walks and increase to 15-minutes over a few weeks. Increase intensity as you gather strength. Build from there. Find sports and activities you like that keep you off the couch and moving. Mow your lawn; clean out the garage; take up tennis or racquet ball. Cut fat in your diet and you will automatically cut calories. Cut down on portion sizes. Check out our home page for different ways to eat and exercise for the way to healthy weight loss. Diet pills may work for a while but not without side effects. For lifetime weight control, the answer is diet and exercise.</p>

			<content:encoded><![CDATA[<p>Weight Loss Surgery: Extreme Medical Intervention for When Lives Are.<br />
For most of us, those extra pounds we need to lose are a hassle &#8211; inconvenient and unwanted, a little pesky reminder of our overindulgences. For some, though, excess weight is not only inconvenient, it is actually life threatening. Weight Loss Surgery (WLS) is offering some of these people a new chance at life.<br />

Who Is a Candidate for Weight Loss Surgery?</p>
<p>Persons who are &#8220;morbidly obese&#8221; (typically defined as having a BMI of 35 or greater, generally meaning at least 100 pounds over their recommended weight range) may be at risk for significant health effects. These include increased risk of hypertension, high blood pressure, heart disease, heart attack, sleep apnea, diabetes, arthritis, back pain and certain forms of cancer. In addition, the excess weight limits the lifestyle of many people, making simple everyday tasks challenging &#8211; and in some cases, impossible.</p>
<p>Weight Loss Surgery is a desperate and permanent attempt to correct what dieting and exercise cannot. Physicians screen their patients rigorously before referring them for WLS &#8211; this is not a diet drug, to be dispensed readily. However, some patients have metabolic issues and cannot lose the weight, either due to their genetics or to the effects of years of restricted dieting, or both. When normal dieting fails, and the patient reaches a point where their everyday lifestyle is dramatically affected and their life is actually at risk because of their weight, WLS may be considered.</p>
<p>What Is Weight Loss Surgery?</p>
<p>Normally, a human will have approximately 20 feet of small intestine (comprised of the ileum and the jejunum). The duodenum connects the stomach to the jejunum, and the ileum feeds into the colon. In the WLS procedure, the surgeon bypasses part or all of the small intestine using surgical staples to form a small pouch out of the stomach.</p>
<p>There are two major types of Weight Loss Surgery, which is also called Bariatric Surgery. Both of these are dramatically different from the old &#8220;stomach stapling&#8221; of 20 years ago. Nonetheless, they are still not without risks.</p>
<p>The first is called a Roux-En-Y Gastric Bypass (RNY) and is the most common technique. In this procedure, the surgeon actually staples off the majority of the stomach area. A small pouch is left intact, and this is connected with the digestive tract, bypassing the duodenum and part of the jejunum. Approximately three to five feet of small intestine are bypassed. In the Fobi Pouch technique, the stomach is left attached. Other variations dissect and remove the unused stomach.</p>
<p>In a far less common procedure, the Extensive Gastric Bypass procedure, almost the entire small intestine is bypassed. Approximately 15 feet of small intestine is bypassed after the surgically segmented into a much smaller pouch, the extra stomach is removed. In a variation of this called the Duodenal Switch, a small portion of the duodenum is left attached to help reduce &#8220;dumping syndrome.&#8221; In general, weight loss is more rapid with the Extensive Gastric Bypass; however, some physicians worry about nutritional deficiencies with this form of the surgery.</p>
<p>Vertical Gastric Banding (VBG), where physical bands are placed to create the pouch but no small intestine is bypassed, is an older technique that is rarely performed anymore. The complications with this procedure are similar to the RNY technique, plus some patients complained about discomfort from the bands themselves.</p>
<p>Some surgeons will perform these procedures laparoscopically (that is, through tiny incisions using large telescopes and long-handled instruments) instead of through the large abdominal incision that is typical. This is a far more advanced procedure and requires additional skill and technique to perform correctly; in addition, patients with adhesions (internal scar tissues) from previous surgeries or infections may not be candidates for laparoscopic surgery. In the end, the best procedure is the one your surgeon is the most skilled at performing!</p>
<p>Recovering From Weight Loss Surgery</p>
<p>Life after WLS can be a huge adjustment. Initially, patients are kept for several days in the hospital. Upon returning home, they must adapt to eating very small meals. Because the stomach is literally only a fraction of its previous size, WLS patients can only eat very small amounts at one time. A steak and potatoes dinner is not only unhealthy for them, but is literally impossible to eat. The stomach pouch initially can only hold two tablespoons of food at one time; it eventually stretches until it can hold ? to 1 cup of food &#8211; still not a lot!</p>
<p>Furthermore, because some of the food passes through the small intestine undigested, and one of the areas that was removed helped finish the digestion and absorption process, the patient literally cannot handle a large volume of food. Overeating will generally result in vomiting, as the patient&#8217;s system is unable to process this volume of food at one time. Because of the body&#8217;s basic metabolic needs, though, these small meals cannot sustain them, so patients must learn to eat more frequent small meals and regular small, healthy snacks.</p>
<p>The foods that are well tolerated by the WLS patient are generally those that are relatively bland, and not excessively sweet or high in fat. Learning what your body will and won&#8217;t tolerate is somewhat of a matter of trial and error; however, a list of healthy alternatives and suggested volumes is provided. Food must be well chewed, especially anything fibrous, as the digestive system is severely shortened. Vitamin supplements are needed to replace the body&#8217;s ability to absorb nutritional vitamins during the digestion process.</p>
<p>There are online support groups as well for WLS patients. Unlike traditional &#8220;diets,&#8221; Weight Loss Surgery is permanent and dramatically lifestyle changing. Those who have gone through the surgery can provide insight into strategies on making the adjustment more smoothly. They can give new WLS advice, tips and hints on proper nutrition, physical recovery, what to expect, etc.</p>
<p>Does it Work?</p>
<p>While WLS is clearly not for everyone, I have four friends who have been through this procedure. All were (prior to their surgery) &#8220;morbidly obese&#8221; and all had been diagnosed as high-risk due to their weight. In every case, the WLS required a significant lifestyle adjustment, both in activity level and in meal planning and preparation.</p>
<p>All four of my friends have lost significant amounts of weight. Two have lost over 100 pounds each, and one is nearing that number. The fourth has lost about 60 pounds but says she has more energy and self-esteem now than she ever remembers having before. Two of the four had fairly long (physical) recoveries, and one was hospitalized with post-surgical complications (which cleared fairly quickly but were of a serious nature at the time).</p>
<p>The weight loss is rapid &#8211; generally patients will lose their weight over 12 &#8211; 24 months. Patients must work with their physicians to monitor their diets, and make sure that their weight loss patterns are normal and healthy for them as individuals. What one person experiences may be totally different than what another experiences, weight loss wise.</p>
<p>All four of my friends who had WLS are able to do physical activities and be involved in their children&#8217;s lives in ways that were never physically possible before, and two of the four have started formal exercise programs. They have commented on their increased energy levels, and all have increased self-esteem levels. They are unanimously enjoying watching their bodies &#8220;shrink!&#8221; One thing to consider is that the changes are relatively dramatic, and that your friends and co-workers will definitely notice the changes (over time, at least). You also will not be able to partake in large social lunches or dinners; you will need to stay on your special diet (quantity wise) for the rest of your life. This may be embarrassing if you are a very private person; however, the potential health benefits of lowered body mass will hopefully help you overcome your embarrassment at leaving most of your lunch untouched. One friend says that she and her husband order a full meal and split it between the two of them (they both had WLS) and still usually have leftovers to take home.</p>
<p>Is Weight Loss Surgery Right for You?</p>
<p>Bariatric surgery should never be considered as a first attempt to lose weight. This is a serious and permanent change, and like any major surgery, has serious and potentially deadly risks associated with it. This is a procedure that should be considered a &#8220;last resort&#8221; &#8211; for persons who are unable to control their weight through normal means (diet, exercise and even prescription medication) and who face serious health risks if their weight issues are not resolved fairly quickly.</p>
<p>Some complications of WLS include development of gallstones, abdominal hernias, development of &#8220;aprons&#8221; (excess skin tissue that hangs loosely on the abdomen), nutritional deficiencies.</p>
<p>If you are considering WLS, I would strongly advise you to research the matter thoroughly. If you have tried exercise, dieting and even medication to no avail, you may want to discuss WLS with your doctor to see if you are a candidate. Talk to your doctor about the pros and cons, and weigh the risks of the surgery and the pain of recovery (and the permanent lifestyle change required) with the risk of keeping the weight on.</p>

<p>1/2 package tofu, creamed</p>
<p>1 cup quick oats</p>
<p>3/4 cup wheat flour</p>
<p>1/4 cup vegetarian beef seasoning</p>
<p>1 tsp. Curry powder</p>
<p>1/4 cup shredded soy cheese</p>
<p>Mix ingredients together. Form into balls and flatten into patties. Fry until browned; transfer to baking sheet and bake in a 350 degree oven for 10-15 minutes.</p>
<p>Use as you would real hamburgers. Makes 6-7 patties.</p>
